Team Registration

ICE HOCKEY TEAM REGISTRATION -Entry fee:  $ 995 Can (including 13% ON Harmonized Sales Tax).  Submit one form per team.
Please complete the form below and a payment web page will be displayed to make a payment through PayPal or pay by cheque: $400 deposit - entrance fee (non-refundable) before March 15th, 2011 required to hold your spot; and a $595 final payment, due 1 MONTH before the date of the event?

Complétez la fiche d'inscription du tournoi ci-dessous et une page Web s'affichera pour payer les frais d'inscription via PayPal ou encore payer par chèque: 400$ dépôt (non remboursable) pour garantir votre participation avant le 15 Mars 2011; et un montant final de 595$ dû 1 MOIS avant la date de l'évènement.
